* All sections of the online application form must be completed.

* Local education agency representatives may also apply for admission for deaf and hard of hearing students in their school districts with the support of the student’s family.

SUBMIT STUDENT RECORDS

Submit the Student Records Request Form to your child’s current school in order to have their records sent to KDES.

ENROLLMENT

Once your child has been accepted, then you will need to submit the following medical forms to the Admissions Office in order to enroll:

Annual Physical, Vaccinations, and Oral Health Assessment (DC Universal Health Certificate)

Tuberculosis (TB) Risk Assessment Questionnaire

Tuberculin (TB) Skin Test as determined by your child's health professional

DC's Department of Health: School Immunization Requirements

DC's Department of Health: Human Papillomavirus (HPV) Vaccination Opt-out Certificate

Medical clearance is required before your child can enroll.
